Gulfside Healthcare Services in Florida is seeking a licensed RN to join the Hospice IPU team. Reporting to the Clinical Care Manager - IPU, the Hospice RN provides high quality skilled care to patients with acute needs and collaborates with LPNs, CNAs, and other staff to maximize comfort and quality of life.

You will perform ongoing assessments, develop and implement care plans, educate families, and participate in the interdisciplinary group.

Formación

• Must be a licensed RN in Florida.
• Minimum of one year nursing experience in acute care preferred.
• Strong ethical standards and professional demeanor.
• Sound critical thinking, time management, technical skills, analytical ability, good judgment.
• Ideal candidate is well-organized, self-directed, and a team player.
• Must have reliable transportation and valid Florida driver’s license.

Responsabilidades

• Case manages patients in the IPU during shift.
• Ongoing assessment of physical, psychological, social, and spiritual needs; initiate intervention.
• Provide or oversee supervision of LPNs, CNAs, and other staff.
• Take independent action to identify and resolve problems.
• Support policies and procedures and orient new staff.
• Mentor and guide coworkers; maintain positive relationships with all disciplines.
• Address patient/family concerns; improve relationships with care facility partners.
• Educate the community about hospice care; engage in community consultations.
• Communicate with the Clinical Care Manager-IPU and monitor risk management.
• Ensure nursing care plans are created, reviewed daily, and updated as needed.
