Job summary

Affinity Hospice Holdings, LLC is seeking an Inpatient Registered Nurse (RN) to provide direct nursing care in an inpatient setting. The role emphasizes high-quality, patient-centered care and collaboration with the interdisciplinary team to ensure safe treatment delivery.

The position requires an RN license, CPR certification, and at least 1 year of acute care experience; hospice/home care experience is preferred. Full-time with night hours; driving may be required.

• Excellent observation, verbal and written communication skills, problem solving skills; nursing skills per competency checklist.
• Strong clinical assessment and monitoring abilities.
• Ability to work with interdisciplinary teams and adapt to changing patient needs.

Responsibilities

• Provide professional nursing care in accordance with the State Nurse Practice Act and hospital policies.
• Conduct initial and ongoing patient assessments, including vital signs, exams, and medical history review.
• Develop, implement, and evaluate individualized care plans based on patient needs.
• Administer medications and treatments as prescribed by physicians.
• Monitor and document patient status, changes in condition, and response to treatments.
• Provide patient and family education regarding disease management, medications, and self-care techniques.
• Assist patients with activities of daily living to promote comfort and independence.
• Collaborate with physicians, therapists, and interdisciplinary team members to coordinate care.
• Ensure patient safety and maintain a therapeutic and supportive environment.
• Participate in daily rounds, interdisciplinary team meetings, and patient care conferences.
• Engage in quality improvement initiatives and maintain regulatory compliance.
• Maintain accurate and timely documentation in the electronic health record.
• Attend continuing education, in-services, and training to maintain clinical competency.
