AccentCare Inc. is seeking a Clinical Liaison VIPC, Inpatient Hospice Admissions RN in Cypress, TX to identify patient needs and coordinate seamless transitions into hospice services.

The role requires one year of RN experience, strong communication, and ability to work independently in a hospital setting, with driving responsibilities. You will build trusted relationships with physicians and care teams, educate on hospice eligibility, and advocate for patients and families during goals‑of‑care

Qualifications

• One year of RN experience.
• Strong communication and relationship-building skills.
• Ability to work independently in a hospital environment.
• Passion for serving with dignity and empathy.
• Valid driver’s license and insured vehicle.

Responsibilities

• Build and maintain trusted relationships with physicians, hospital staff, and care teams.
• Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
• Conduct patient assessments and coordinate the admission process.
• Partner with Admissions, Clinical, and Care Teams for smooth transitions.
• Advocate for patients and families during goals‑of‑care conversations.
• Participate in interdisciplinary planning, training, and outreach.
• Uphold ethics, quality, and continuous learning.
• Meet expectations for response times, referral coordination, and communication.
