Job summary

AccentCare Inc. is seeking a Clinical Liaison VIPC, Inpatient Hospice Admissions RN to support patients and families during transitions to hospice care. The role focuses on identifying patient needs, education, and coordinating admissions with hospital teams, physicians, and the hospice staff.

The position is PRN, weekend heavy (Sat/Sun 8am-5pm) with some flexibility. Requires active RN license, strong communication, and the ability to work independently in a hospital environment.

Qualifications

• One year of RN experience with knowledge of home health regulations, Medicare requirements, and care-plan development.
• Medical sales or clinical outreach experience preferred.
• Strong communication and relationship-building skills.
• Ability to work independently in a hospital environment.
• Passion for serving with dignity and empathy.
• Active RN license.
• Valid driver's license and insured vehicle.

Responsibilities

• Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily in-person collaboration and communication.
• Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
• Conduct patient assessments and coordinate the admission process.
• Partner with Admissions, Clinical, and Care Teams for smooth transitions.
• Advocate for patients and families during goals-of-care conversations.
• Participate in interdisciplinary planning, training, and outreach.
• Uphold ethics, quality, and continuous learning.
• Meet expectations for response times, referral coordination, and communication.
