AccentCare is seeking a Clinical Liaison VIPC, Inpatient Hospice Admissions RN in Houston. The role identifies patient needs and facilitates transitions into hospice services, serving as the clinical resource for education, eligibility, and admissions.

You will build relationships with physicians and hospital staff, coordinate admissions, and advocate for patients and families during goals‑of‑care conversations. Weekend schedule Fri/Sat/Sun 10a‑10p emphasizes compassionate, collaborative care.

Qualifications

• One year of RN experience with knowledge of home health regulations, Medicare requirements, and care‑plan development
• Medical sales or clinical outreach experience preferred
• Strong communication and relationship‑building skills
• Ability to work independently in a hospital environment
• Active RN license and valid driver’s license with insured vehicle

Responsibilities

• Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily in‑person collaboration and communication.
• Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
• Conduct patient assessments and coordinate the admission process.
• Partner with Admissions, Clinical, and Care Teams for smooth transitions.
• Advocate for patients and families during goals‑of‑care conversations.
• Participate in interdisciplinary planning, training, and outreach.
• Uphold ethics, quality, and continuous learning.
• Meet expectations for response times, referral coordination, and communication.
