Job summary

Qualifications

Responsibilities

• Provide and coordinate clinically competent care for an assigned patient group.
• Document data collection and adjust treatment plans as indicated.
• Educate patients and families about learning needs and care plans.
• Direct and coordinate communication with dialysis teams and facility staff.
• Administer medications and respond to adverse reactions per protocol.
• Participate in staff training and on-call activities as assigned.
