Job summary

Stony Brook Medicine seeks an Inpatient Diabetes Educator (CDCES) to provide state-of-the-art diabetes education and self-management support for adults in the inpatient setting. You will assess, plan, implement and evaluate patient-centered DSME programs, collaborating with the medical team to meet Joint Commission standards.

The role requires NYS RN licensure and CDCES credential, with 2–5 years in diabetes program management. Per diem schedule and Per Diem department alignment.

Qualifications

• RN with current NYS license.
• CDCES with experience teaching comprehensive diabetes self-management education.
• 2-5 years experience specializing in Diabetes program management and development, research experience, and quality assurance experience.

Responsibilities

• Provide 1:1 DSMES in inpatient setting.
• Communicate with referring primary medical team.
• Lead updating of DSME materials to maintain certification standards.
• Plan, teach, and evaluate the delivery of DSME for adult patients with diabetes.
