Job summary

Stony Brook University in New York seeks an Inservice Educator II to provide state-of-the-art CDCES diabetes education assessment and DSME to patients and families at Stony Brook Medicine. The role collaborates with the medical team to plan and evaluate patient-centered diabetes education programs, maintains records, and supports ongoing certification, with a focus on inpatient care.

Requires NYS RN license; CDCES within 12 months; prior diabetes program management experience preferred;

Qualifications

• Active NYS RN license required.
• CDCES certification must be obtained within 12 months of hire.
• Experience with diabetes self-management education (DSME) preferred.
• 2–5 years' diabetes program management experience preferred.
• Knowledge of ADA, AACE, and ADA Educators guidelines (standards).

Responsibilities

• Provide 1:1 inpatient DSMES.
• Coordinate communication with referring medical teams.
• Support nursing orientation and critical care diabetes education.
• Participate as an active member of the Diabetes Advisory Committee.
• Maintain documentation and ensure certification readiness.
