Job summary

Johns Hopkins Hospital in Baltimore seeks a Certified Diabetes Care & Education Specialist (Dietitian or Nurse) to educate patients and families in an inpatient setting. You will complete comprehensive assessments and develop personalized care plans, implementing the Diabetes Education Program with insulin pumps and CGMs.

Qualifications

• Bachelor's degree in Dietetics or Nursing.
• Current license or registration in allied health (e.g. RN or RD).
• Must obtain CDCES within 1 year of employment.
• Strong knowledge of medical terminology and disease processes.
• Proficient in MS PowerPoint, Excel, Outlook and EMR documentation.

Responsibilities

• Provide diabetes education to patients and families in inpatient setting.
• Complete comprehensive diabetes assessments and develop individualized care plans.
• Implement Johns Hopkins Medicine Diabetes Education Program including insulin pump therapy and CGMs.
• Collaborate with physicians, advanced practice providers and social workers to ensure continuity of care.
