Job summary

Fulton County Health Center in Ohio seeks an inpatient coder/abstractor to join the Health Information team. This role codes and abstracts records across hospital services, ensuring data integrity, regulatory compliance, and timely electronic data entry.

The ideal candidate maintains accurate patient information and supports clinical and statistical reporting, with RHIT/CCS credentials and familiarity with MEDITECH and encoder software.

Qualifications

• RHIT or CCS credential required; AHIMA credentialing in progress may be considered if completion expected within six months of hire.
• Ongoing participation in continuing education and professional development is expected.
• Proficiency with MEDITECH, Microsoft Word, Excel, PowerPoint, and encoder software (3M preferred).
• Knowledge of ICD-10-CM/PCS, CPT-4, UHDDS guidelines, and Critical Access Hospital coding requirements.
• Familiarity with medical terminology, anatomy, physiology, procedures, and clinical documentation.
• Ability to accurately abstract clinical information and communicate data clearly.
• Strong attention to detail with excellent analytical and organizational skills.

Responsibilities

• Review records and assign diagnosis and procedure codes using ICD-10-CM/PCS and CPT-4.
• Abstract and sequence clinical data from medical records per UHDDS guidelines.
• Perform chart analysis to ensure documentation is complete, accurate, and compliant.
• Generate clinical and statistical reports and respond to internal data requests.
• Maintain compliance with coding policies and procedures.
• Support quality assurance and performance improvement through accurate data.
• Identify incomplete or inconsistent documentation and communicate findings.
• Assist with Master Patient Index maintenance and accurate patient identification.
