Job summary

UVMMC is seeking a Clinical Staff Pharmacist for an inpatient, full-time role in Burlington, VT. The pharmacist will participate in antimicrobial stewardship, medication safety, and dosing optimization as part of a multidisciplinary care team.

Responsibilities include order verification, pharmacokinetic monitoring, IV to PO conversions, and therapeutic interchange. The role may involve precepting ASHP residents and students during rounds and education sessions.

Qualifications

• Bachelor of Science in Pharmacy from an ACPE-accredited school.
• Doctor of Pharmacy degree preferred.
• ASHP-accredited Residency or equivalent experience preferred.
• Licensed Pharmacist in Vermont.

Responsibilities

• Perform order verification and assess drug therapy for safety and efficacy.
• Apply pharmacokinetic principles to adjust dosing.
• IV to PO conversions; conduct therapeutic interchange as appropriate.
• Participate in multidisciplinary rounds and patient care initiatives.
• Serve as a preceptor to residents and students.
