UVMMC inpatient pharmacy operates 24/7; Clinical Staff Pharmacists support antimicrobial stewardship and medication safety. You will review orders, optimize dosing, and participate in rounds with multidisciplinary teams to ensure patient- centered care.

Role includes order verification, pharmacokinetics, IV-to-PO conversions, therapeutic interchanges, and potential precepting of pharmacy residents in a dynamic hospital setting.

Formación

• BS in Pharmacy from ACPE-accredited school.
• Pharmacy license in Vermont required.
• DOPharmacy degree preferred (PharmD).
• Residency or equivalent experience preferred.

Responsabilidades

• Perform order verification and provide drug information.
• Apply pharmacokinetics and adjust dosing as needed.
• Manage IV to PO transitions and therapeutic interchanges.
• Provide education/training and serve as a preceptor.
