Join Cleveland Clinic Mercy Hospital as a Clinical Pharmacist – Inpatient, supporting 24/7 pharmacy operations and collaborating with infusion and oncology teams. Responsibilities include order verification, IV room support, education, and medication histories to optimize patient care.

The role involves day and evening shifts with weekend rotation and holidays. Minimum qualifications include PharmD or BS in Pharmacy, active RPh licensure or reciprocity eligibility, and required certifications;

Qualifications

• PharmD or BS in Pharmacy from an accredited college of pharmacy.
• Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation within 90 days of hire.
• Basic Life Support (BLS) certification and immunization administration training within 90 days.
• ACLS and/or PALS certification required within one year of hire.
• Board certification (preferred).
• Post-graduate year 1 residency (PGY1) or relevant PGY2 experience (preferred).

Responsabilités

• Provide safe, efficient, patient-centered, evidence-based medication services.
• Verify medication orders and dispense medications in accordance with regulations.
• Document services completely and accurately in the medical record.
• Ensure pharmacy operations comply with national accreditation standards.
• Maintain professional interactions with patients to facilitate positive experiences.
• Educate and train students, residents, other caregivers, and patients.
