Job summary

Regional One Health invites an experienced Clinical Pharmacist to join our Inpatient Pharmacy team. The role focuses on delivering clinical services, drug information, and therapeutic review to medical staff and patients.

The incumbent will work closely with physicians and nurses to optimize drug therapy, monitor for adverse reactions, and participate in formulary and P&T activities. TN license and PharmD are required; PGY1 residency is preferred.

Qualifications

• Pharm D degree and TN licensure as a pharmacist.
• Experience in inpatient clinical pharmacy preferred.

Responsibilities

• Provide daily clinical services and ensure accurate drug therapy for patients.
• Review patient records and monitor for interactions, contraindications, allergies and adverse reactions.
• Communicate with physicians and nurses about patient drug therapy questions.
• Provide pharmacotherapy consults and consultative services to patients.
