Job summary

Regional One Health in Memphis, TN is seeking a Clinical Pharmacist for our Outpatient Specialty Pharmacy (Internal Medicine). You will provide clinical services, review drug therapy and collaborate with medical staff to optimize patient care.

Responsibilities include pharmacotherapy consults, medication reviews, ensuring safe, effective, and cost-efficient regimens, and participating in quality improvement initiatives. Eligible candidates will hold a PharmD and TN licensure.

Qualifications

• PharmD degree from an accredited school.
• Licensed Pharmacist in Tennessee upon hire.
• Experience in clinical pharmacy practices and patient care preferred.

Responsibilities

• Provide clinical services to assigned patients daily.
• Review patient records for drug therapy optimization.
• Coordinate with physicians and nurses on patient drug therapy questions.
• Perform pharmacotherapy consults and provide consultation to patients.
