Qualifications

• Pharmacist degree required: Doctorate of Pharmacy or Bachelor of Science in Pharmacy with 2 years hospital pharmacy experience.
• State license to practice pharmacy required.
• Preferred: 1-2 years clinical pharmacy experience; completion of a postgraduate residency.

Responsibilities

• Dispense medications according to procedures and regulations; optimize therapy and patient outcomes.
• Provide consultation on non-formulary drug requests and counsel patients regarding dosage, side effects and interactions.
• Monitor inpatient medication profiles for interactions and ensure proper storage and handling.
• Prepare, label and ensure aseptic technique for medications.
• Supervise and mentor PharmD students and residents; participate in program evaluations.
