Qualifications

• Bachelor's Degree in Pharmacy from an accredited school of Pharmacy.
• State licensure as a Pharmacist and any relevant life support certification.

Responsibilities

• Reviews, interprets and evaluates medication regimens to identify, prevent and resolve potential or actual medication-related problems.
• Initiates, modifies, and monitors medication therapy through approved mechanisms.
• Provides drug information and education to medical professionals, students, and patients.
• Collaborates with care team to develop, implement and evaluate medication management quality improvement efforts.
• Ability to work in a fast-paced environment with decisive judgment and minimal supervision.
