Job summary

The Mayo Clinic is seeking a pharmacist to provide medication management in coordination with other health care professionals to deliver safe, appropriate, and cost-effective therapy to patients.

Responsibilities include assessment of medication therapy regimens, MTM, order verification/entry, distribution, monitoring, documentation of therapy plans, and education, with active participation in quality improvement and research activities.

Job description

Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies.
