Qualifications

• Doctor of Pharmacy (Pharm.D.) from ACPE-accredited program or Bachelor of Science with at least 2 years’ relevant experience.
• Current pharmacist license in the practice state or eligible to obtain per site policy.
• Evidence of ongoing scholarly pursuits (research/quality improvement/publications) preferred.
• Commitment to excellence, accuracy, attention to detail and teamwork.

Responsibilities

• Provide medication management services in coordination with health care professionals to deliver safe, cost-effective therapy.
