Job summary

Qualifications

• PharmD or BS in Pharmacy from an accredited program.
• Active, unrestricted Pharmacist license in the state of practice.
• Minimum 1 year inpatient or hospital pharmacy experience preferred.
• Strong knowledge of pharmacotherapy, medication safety, and pharmacy law.
• Experience with pharmacy information systems and EMR preferred.

Responsibilities

• Review, verify, and dispense medications according to physician orders and hospital protocols.
• Evaluate medication orders for dosing, interactions, allergies, contraindications, and duplications.
• Provide clinical pharmacy recommendations to physicians and other healthcare professionals.
• Monitor medication therapy, therapeutic outcomes, and potential adverse reactions.
• Prepare and oversee sterile and non-sterile medications as required.
• Manage IV medications, controlled substances, high-alert medications, and specialty therapies.
• Participate in medication reconciliation, antimicrobial stewardship, and clinical pharmacy initiatives.
• Provide medication counseling and education to patients and healthcare staff when appropriate.
• Maintain accurate medication records and documentation in the pharmacy system.
• Ensure compliance with state and federal pharmacy regulations, USP standards, and hospital policies.
• Support medication safety, quality improvement, and pharmacy operations.

Required Qualifications:

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited program

• Strong knowledge of pharmacotherapy, medication safety, and pharmacy law
• Experience with pharmacy information systems and electronic medical records (EMR) preferred
• Excellent clinical, communication, organizational, and problem-solving skills
• Ability to work effectively in a fast-paced hospital environment
