Lourdes Behavioral Health in Pasco, WA, part of Lifepoint Health, seeks a Social Worker to join our team. You’ll be integral to care coordination, discharge planning, and collaboration with interdisciplinary teams to support patient well-being and safe transitions.

Formación

• Effective oral and written communication skills in English.
• Currently Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing preferred; for example, CCM, ARN, ACM or ANCC RN-BC.
• Minimum of 2 years social work or case management experience in an inpatient setting highly preferred.

Responsabilidades

• Coordinate weekly care team conferences to refine treatment plans and achieve patient goals.
• Review CMG and identify potential missed comorbid conditions for the patient's stay.
• Assist with utilization review activities, denials and appeals; coordinate physician peer reviews with payers.
• Provide clinical updates to insurers and document payer communications in Meditech.
• Coordinate discharge planning, home health, follow-up care, equipment, therapies and hospice services.
• Identify trends affecting quality, cost, patient experience and care delivery; report to leadership.
