Job summary

Lourdes Behavioral Health is seeking a Case Manager, Social Worker to join our team. You will interview and assess patients and families, coordinate services, participate in case reviews, and assist with discharge planning to promote optimal patient outcomes.

We emphasize collaboration with the healthcare team and supporting patients through referrals to needed resources, including transportation, housing, and social services.

Qualifications

• Bachelor's Degree or equivalent required.
• Licensed Social Worker within six months of hire/transfer date.
• Basic Life Support (BLS) certification within 30 days of hire date.

Responsibilities

• Interviews and assesses patients and/or family, caregivers, or legal representatives.
• Determines, prioritizes, provides and/or arranges needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress.
• Collaborates with healthcare team to ensure adherence to treatment plan; assists with discharge planning.
