Job summary

Astrana Health, Inc. is seeking a Case Manager (RN/LVN) to coordinate care for adult patients and support safe transitions of care. You will collaborate with hospitalists, discharge planners, and care teams to optimize utilization and reduce hospital readmissions.

Qualifications

• Active Rhode Island RN or LVN license is required.
• Minimum 2 years of clinical nursing experience.
• Ability to commute to Johnston, RI.
• Strong analytical and critical reasoning abilities.

Responsibilities

• Comply with Case Management Department policies and procedures.
• Achieve compliance with Utilization Management standards for Case Management.
• Collaborate with hospitalists, case managers, and discharge planners to establish discharge plans.
• Review medical records to determine medical necessity and level of care.
• Assess, plan, implement, coordinate, monitor, and evaluate services to reduce readmissions.
• Report daily activities and trends to Case Management leadership.
