Responsibilities

• Coordinate comprehensive care management for patients across the healthcare continuum.
• Support in-hospital patients identified in need of case management.
• Promote effective use and monitoring of health services; collaborate with care team and patient/caregiver to manage transitions.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
