Sentara Northern Virginia Medical Center in Woodbridge, VA is hiring an Inpatient Case Manager – RN for the day shift (Mon-Fri 8:00am-4:30pm, rotating weekends/holidays). This role manages comprehensive care plans, coordinates transitions, and ensures clinical appropriateness and optimal LOS across hospital services.

Required: Master’s in Social Work (MSW) and Case Management certification within a year; BLS within 90 days. Prior acute care experience preferred with strong collaboration skills.

• Certification in Case Management (CCM, CMAC, or ACM-SW) required within one year of eligibility.
• 1 year of acute care/healthcare experience required; internship in acute care acceptable.
• BLS required within 90 days of hire.

Directly support those in-hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver.
