North Country Healthcare is seeking an Inpatient Case Manager to lead comprehensive patient care coordination during hospitalization, ensuring safe discharge and addressing medical, psychosocial, and financial barriers. This role emphasizes rural settings and requires clinical judgment and interdisciplinary collaboration.

The position focuses on admission assessments, discharge planning, resource navigation, and advocacy to optimize patient outcomes while maintaining safety and quality

Qualifications

• Associate's degree or higher in a relevant field required or 4 years of equivalent experience.
• Nurse or Social Worker preferred; LNA/MA with experience considered.
• Certification in Case Management (CCM) preferred; BLS/ACLS/PALS as required.

Responsibilities

• Comprehensive assessment of patients’ medical, psychosocial, and financial needs on admission.
• Identify barriers to discharge and develop individualized care plans with the team.
• Coordinate with physicians, nurses, and therapists for cohesive care delivery.
• Determine appropriate level of care and plan transitions to avoid readmissions.
• Navigate community resources and financial assistance for post‑acute needs.
• Document activities for compliance and quality reporting; educate patients and families.
