North Country Healthcare is seeking an Inpatient Case Manager to lead patient care coordination during hospitalization, ensuring safe, timely discharge and addressing medical, psychosocial, and financial barriers in a rural New Hampshire setting.

This role requires clinical judgment and collaboration with interdisciplinary teams to optimize outcomes and continuity of care, with an hourly range of $27.27/hr to $36.83/hr. Final compensation will reflect experience and qualifications.

Qualifications

• Bachelor's in Social Work or related field required.
• CCM or equivalent certification preferred.
• CPR/ACLS/PALS certification as applicable.
• Strong communication and problem-solving skills.
• Experience in inpatient case management or discharge planning.

Responsibilities

• Perform in-depth assessments of patients’ medical, psychosocial, and financial needs upon admission.
• Identify barriers to discharge (transportation, home support, insurance, medication access).
• Develop individualized care and discharge plans with the interdisciplinary team.
• Determine appropriate level of care (inpatient, observation, rehab, home health, SNF).
• Coordinate with physicians, nurses, therapists, and external providers for cohesive care.
• Make recommendations to facilitate safe transitions and prevent readmissions.
• Connect patients with community resources and financial assistance programs.
• Educate patients and caregivers on treatment plans, meds, and follow-up care.
• Document all case management activities for compliance and quality reporting.
