Job summary

RN.com is seeking a skilled RN Case Manager to provide comprehensive case management for inpatient and observation patients. You will assess psychosocial, medical, and discharge needs, coordinate the care plan, and drive the discharge plan with the multidisciplinary team.

Responsibilities include using InterQual to evaluate care progression and ensure timely, appropriate discharge planning. This role requires 2 years of case management in acute settings and an active TX/compact RN license.

Qualifications

• RN license (TX/compact) or compact state license required.
• BLS certification required.
• At least 2 years of case management experience in an acute hospital setting.
• Acute hospital experience within the last year with no more than a 90-day break.

Responsibilities

• Perform comprehensive assessment of psychosocial, medical and discharge needs for patients and families.
• Coordinate the plan of care and drive the discharge plan in collaboration with the multidisciplinary team.
• Evaluate progression of care using InterQual criteria throughout the episode of care.
