White Oak Medical Center invites an experienced RN or SW to join our Care Navigation department as a Care Navigator/Discharge Planner. The role focuses on psychosocial assessments, discharge planning, and cross-disciplinary collaboration to ensure safe transitions of care.

The position requires RN licensure or SW credentials, BLS, and 1–3 years of clinical experience. Cerner EMR and All Scripts experience are preferred to support efficient care coordination.

Formación

• Graduate of an accredited nursing program with an associate degree as minimum; bachelor’s degree preferred.
• RN or SW credentialing as applicable (RN licensed; SW with required degrees).
• BLS certification required.

Responsabilidades

• Conduct comprehensive psychosocial assessment focusing on transition needs.
• Collaborate with physicians, nurses, social workers to coordinate discharge planning.
• Identify barriers to discharge and communicate with leadership for resolution.
• Care for high‑risk patients, navigate discharge plans and arrange follow‑ups.
• Act as liaison among patients, caregivers and multidisciplinary providers.
