The RN requires nursing licensure and 1–3 years of clinical experience; SW requires BS/W or MSW with 3+ years in diverse clinical settings. BLS, Cerner EMR, and Allscripts experience preferred.

Qualifications

• RN or SW role requires degree/higher licensure and relevant clinical experience.
• RN: AS degree required, BSN preferred; SW: BSW/MSW with 3+ years preferred.
• BLS certification required; Cerner EMR and Allscripts experience preferred.

Responsibilities

• Perform psycho-social assessments and assess patient discharge needs.
• Coordinate with physicians, nurses and social workers for discharge planning.
• Navigate testing and treatment to prevent discharge delays; report barriers.
• Support high-risk patients with discharge planning and follow-up.
• Educate patients and families on diagnoses, options, and resources.
