Job summary

Central Ohio Primary Care is seeking an Inpatient Support Center RN in Westerville, OH. The part-time role offers benefits and requires 3+ years in acute care, EPIC experience, and strong patient-centered communication.

The RN will triage, support hospital teams, and ensure smooth admissions, transfers, and continuity of care while maintaining confidentiality and teamwork with physicians and staff.

Qualifications

• Active Ohio RN license is required.
• BSN preferred.
• Experience with EPIC EHR is preferred.

Responsibilities

• Provide clinical support for hospitalized COPC patients.
• Triage and support hospital care teams and providers.
• Maintain up-to-date knowledge of processes and COPC resources.
• Communicate effectively with ED, transfer centers, PCP offices and partners.
• Support continuity of care and patient lists for Hospitalists.
