Job summary

Qualifications

• Knowledge of evidence-based practice and disease-specific processes; technology support and payer protocols desired.

Responsibilities

• Coordinate care across inpatient and outpatient settings to ensure smooth transitions.
• Collaborate with physicians, nurses, and other health care workers to plan and follow up on care.
• Monitor patient care processes and document data to improve timeliness and quality of care.
• Support discharge planning and transition to outpatient care.
