Qualifications

  • Strong teamwork and patient advocacy skills are essential.

Responsibilities

  • Get to know the patient and their needs by completing assessments and planning a care approach.
  • Coordinate care from admission through discharge, connecting patients with internal and community resources.
  • Work with an interdisciplinary team to evaluate needs and ensure appropriate level of care.
  • Assist patients and families with discharge planning, education, referrals, and transition logistics.
  • Review medical records to identify barriers, gaps in service, or concerns about outcomes and costs.

Job Description

As an Inpatient Care Manager, you'll work closely with patients, families, caregivers, providers, and our multidisciplinary care teams to identify barriers to care and connect patients with the resources and services they need. From admission through discharge, you'll help coordinate care, advocate for patients, and make sure the right plan is in place for a safe and successful transition home or to the next level of care.

What You'll Do
  • Get to know the patient and their unique needs by completing assessments, identifying barriers to care, and working with patients and families to develop an appropriate plan.
  • Coordinate care from admission through discharge, connecting patients with internal services, community resources, support programs, and other appropriate services.
  • Work alongside an interdisciplinary team of providers, nurses, therapists, social workers, and other care professionals to evaluate patient needs and coordinate the right level of care.
  • Help patients and families navigate what's next, including discharge planning, education, referrals, and resources that support a safe transition home or to another care setting.
  • Review medical records and patient information to identify potential barriers, gaps in service, or concerns related to outcomes, satisfaction, compliance, cost, and reimbursement.

  • Licensed Clinical Social Worker (LCSW)
  • Registered Nurse (RN)
  • Physical Therapist (PT)
  • Occupational Therapist (OT)
  • Licensed Practical Nurse (LPN)
  • Certification as a Case Manager (CCM) through the Commission for Case Manager Certification (CCMC) or Accredited Case Manager (ACM) through the American Case Management Association (ACMA) is preferred.

Compensation Information

$27.17 - $47.55

The hourly pay range shown represents the base pay range for this position. Individual pay is determined based on factors such as experience, qualifications, skills, and market considerations. In addition to base pay, eligible team members may earn additional compensation through shift, weekend, holiday, on-call, and other applicable differentials.
