Job summary

MaineGeneral Health System seeks a dedicated professional to provide education, advocacy and support to patients and families. You will manage casework and coordinate services across educational, social and advocacy needs, and develop materials tailored to diverse learning styles.

You will link patients to community resources, collaborate with staff, payers and external partners to improve outcomes, and may operate a vehicle for health-system business as needed.

Provides case management and coordination for patients and/or families needing a variety of educational, social, and advocacy services.

Researches, develops, and delivers educational materials related to patient and family support based on learning styles and needs of populations served.

Provides assistance to patients in developing support systems, linking patients and families to community resources as appropriate.

Works collaboratively with staff, payers, and external resources to identify needs, cultivate relationships with internal/external resources, and enhance clinical outcomes.

Some employees in this job may operate a motor vehicle to conduct MaineGeneral Health business

Compensation Information:

$26.50 - $46.38

The hourly pay range shown represents the base pay range for this position. Individual pay is determined based on factors such as experience, qualifications, skills, and market considerations. In addition to base pay, eligible team members may earn additional compensation through shift, weekend, holiday, on-call, and other applicable differentials.
