Job summary

Avera Heart Hospital in Sioux Falls, SD is seeking an Advance Practice Provider (APP) to join a collaborative hospital team. The APP conducts rounds, performs focused assessments, coordinates and evaluates the plan of care, and assists with discharge planning under the direction of the Primary Physician.

Eligible candidates include Certified Physician Assistants with ARC-PA accreditation or Certified Nurse Practitioners with a master’s degree and SD licensure; current BLS/ACLS is required, with

Qualifications

• PA: graduate from ARC-PA accredited PA program.
• CNP: Master’s degree in nursing and SD licensure.
• BLS and ACLS required; NCCPA/NP certifications as applicable.

Responsibilities

• Conduct rounds and patient assessments.
• Coordinate and evaluate plan of care under physician supervision.
• Assist with procedures and administer medications as directed.
• Coordinate discharge planning with care team.
