Job summary

Avera Heart Hospital in Mitchell, SD is seeking an Advanced Practice Provider (NP/PA) to deliver outpatient general cardiology care for established patients in Mitchell and nearby communities. The APP will assess recheck patients, assist cardiologists in outreach clinics, and perform follow-up visits after treatment plans are set.

Applicants should be graduates of an accredited NP or PA program with SD licensure (or eligibility).

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current South Dakota licensure as a NP or PA is required or eligible.

Responsibilities

• Provide outpatient general cardiology care for established patients.
• Assess and manage recheck patients and support cardiologists during Mitchell outreach clinics.
• Independently conduct follow-up outreach visits after treatment plans are established.
• Review diagnostic results, educate patients, and promote coordinated cardiovascular care.
