Job summary

Saint Mary of Nazareth Hospital in Chicago, IL is seeking a Part-time Behavioral Health clinician for the Outpatient department. The role runs weekdays from 10:30 am to 7:00 pm, no weekends, and involves emotional support, crisis intervention, counseling, and referrals within an interdisciplinary team.

Responsibilities include psychosocial assessments, treatment planning, discharge coordination, and accurate documentation.

Qualifications

• Bachelor's degree in Social Work, Marriage and Family Therapy or related field required.
• Master’s in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor or related field, preferred.
• Current registration an intern with Board, as appropriate to education and licensing requirements.
• Current BLS certificate upon hire and maintain current.
• Minimum one (1) year Inpatient or Outpatient Behavioral Health experience preferred.

Responsibilities

• Provide emotional support, crisis intervention and counseling with information/referral services to patients and families.
• Perform case management, milieu management, psychosocial assessments and assist in development and re-evaluation of Treatment Plans as part of an interdisciplinary team.
• Coordinate discharge planning and complete documentation; maintain clinical records for length of stay.
• Complete necessary documentation and maintain clinical records for each client during length of stay.
