Qualifications

• Master's in social work required.
• Licensed Social Worker in Michigan (LLMSW or LMSW) required.
• Experience with diagnostic developmental assessments preferred.
• Demonstrated ability to document reliably and work within an interdisciplinary team.
• Ability to manage a caseload and complete timely records.

Responsibilities

• Performs social service admissions with comprehensive psychosocial assessments.
• Participates in individualized patient care planning with patients and families.
• Attends care conferences and interprets psychosocial needs for care plans.
• Completes mandatory documentation within regulatory timelines.
• Provides linkage to community resources and referrals.
• Plans discharges/transfers to ensure continuity of care.
• Emphasizes safety, patient rights, and person-centered planning.
• Collaborates with other clinical services and Medical Staff.
• Represents the Center in community activities and education programs.
• Maintains licensure CEUs as required by licensing board.
