Job summary

Intermountain Health St George Regional Hospital in Utah is seeking a Behavioral Health Patient Navigator to work on the inpatient psychiatric unit. The role focuses on educating patients about accessing mental health resources and coordinating discharge plans with referrals from Psychiatry, Emergency Department, and other units.

You will connect patients with community services, initiate follow-up after discharge, document plans, and participate in meetings to ensure continuity of care.

Qualifications

• Meets with patients in need of behavioral health services who have been referred from inpatient units, Emergency Departments, and Access Centers to review their discharge plan.
• Assists patients in connecting with referred resources.
• Initiates timely follow up calls with the patient after discharge to ensure adherence to the plan.
• Gathers needed data and completes required logs timely and accurately.
