Qualifications

• Bachelor’s Degree in related field or Bachelor’s in any field with 1 year Human Services experience.
• MHRT Certification (or eligible to obtain) in a reasonable amount of time.
• Valid driver’s license with access to insured vehicle.
• Eligible for Sign On Bonus.

Responsibilities

• Coordinates access to preventive and health services, mental health and substance abuse treatment, and care coordination.
• Meet with clients via Zoom or in home/community settings.
• Build therapeutic rapport with diverse clients and adapt communication style.
• Coordinate and facilitate team meetings with client care team.
• Maintain documentation in electronic medical records per policy and crisis plans.
