Job summary

Horizon Health is seeking a Clinical Assessment Coordinator to oversee the Behavioral Health unit at Fort Duncan Hospital in Eagle Pass, TX. This inpatient psychiatric unit has 18 beds for adults and seniors.

The role includes referral development, pre-admission assessments, and complex evaluations conducted by a Nursing RN, LPC, or LMSW. You will perform on- and off-site assessments, complete admission paperwork, collaborate with the treatment team, and maintain HorizonPlus data for monthly

Qualifications

• Bachelor's degree in nursing is required.
• RN, LPC, or LMSW license required.
• 3-5 years of intake experience required.
• Bilingual with Spanish is required.

Responsibilities

• Diagnose, implements, coordinates and evaluates clinical admission, referral and/or discharge processes.
• Conduct on- and off-site patient assessments, including appropriate test administration, interpretation and recommendations; respond to intake calls and complete admission paperwork.
• Develop treatment plans in coordination with the Treatment team; coordinate treatment with physicians and other health and social agencies; participate as an active member of the interdisciplinary team.
• Enter required data into HorizonPlus system and generate monthly reports to track and trend referral, admission and discharge data.
