Job summary

Horizon Health, the Behavioral Health division, seeks a Clinical Assessment Coordinator (Nursing/RN Registered, LPC, or LMSW) to oversee the 18-bed inpatient psychiatric unit at Fort Duncan Hospital in Eagle Pass, TX. You will manage referral development, pre-admission assessments, and provide specialized evaluations to support treatment planning.

The role requires bilingual skills in Spanish, a Bachelor’s degree in nursing, and an active RN, LPC, or LMSW license, with 3–5 years of intake

Qualifications

• Bilingual with Spanish is required.
• RN, LPC, or LMSW license required.
• 3-5 years of intake experience required.

Responsibilities

• Diagnose, implements, coordinates and evaluates clinical admission, referral and/or discharge processes.
• Conducts on and off-site patient assessments, including appropriate test administration, interpretation and recommendations.
• Participates in the development of treatment plans with the Treatment team and coordinates with physicians and social agencies.
• Enters required data into HorizonPlus and generates monthly reports.
