Job summary

Horizon Health Behavioral Health Services, a Horizon subsidiary, seeks a Clinical Assessment Coordinator for the 17-bed adult/geriatric inpatient psych unit in Ashtabula, OH. The role completes referrals, pre-admission assessments, and specialized evaluations to develop treatment plans.

Required are RN and LPC/LCSW/LMSW credentials, 3–5 years of related experience, and strong data/reporting skills. In this full-time position, you’ll collaborate with treatment teams and court personnel as needed.

Qualifications

• Master’s degree required.
• RN, LPC, LCSW or LMSW required.
• 3–5 years of related experience.
• Proficient in software applications.

Responsibilities

• Diagnose, implement, coordinate and evaluate clinical admission and discharge processes.
• Conduct on- and off-site patient assessments with tests and recommendations.
• Develop treatment plans with the interdisciplinary team and coordinate with physicians and facilities.
