Responsibilities

  • Diagnose, implement, coordinate and evaluate clinical admission, referral and/or discharge processes.
  • Conduct on and off-site patient assessments, including test administration and recommendations.
  • Participate in development of treatment plans with the Treatment team and coordinate with physicians and other agencies.
  • Utilize assessment tools under supervision.
  • Communicate with referral sources including family members, legal system, psychiatrists, and primary care physicians.
  • Review newly admitted patient accounts for eligibility and authorization daily.
  • Maintain schedule for filing court paperwork and hearings; work with court personnel.
  • Enter data into HorizonPlus; generate monthly reports on referrals/admissions/discharges.
  • Prepare program’s key statistical reports and respond to information requests from leadership.
  • Perform other duties as assigned.

Responsibilities

Clinical Assessment Coordinator - RN/ Social Work (RN, LPC, LCSW or LMSW required) - Behavioral Health – Ashtabula, OH.

Horizon Health together with Ashtabula Regional Medical Center is seeking Clinical Assessment Coordinator for the inpatient behavioral health unit in Ashtabula, OH. This is for our 17 bed adult/ geriatric inpatient psych unit. The Clinical Assessment Coordinator completes the referral development and pre-admission patient assessment process and provide clinical assessments for potential clients. A MA/MSW Clinical Assessment Coordinator will complete specialized clinical evaluations, diagnose behavioral health conditions and develop moderate to complex treatment plans.

Responsibilities
  • Diagnose, implements, coordinates and evaluates clinical admission, referral and/or discharge processes.
  • Conducts on and off-site patient assessments, including appropriate test administration, interpretation and recommendations. Responds to intake calls and completes admission paperwork.
  • Participates in the development of treatment plans in coordination with the Treatment team. Coordinates treatment with physicians and other health and social agencies and participates as an active member of the interdisciplinary team.
  • Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, LUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW.
  • Communicates with referral sources including family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
  • Reviews newly admitted patient accounts for eligibility and authorization daily.
  • Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships.
  • Enters required data into HorizonPlus system and generates monthly reports to track and trend referral, admission and discharge data.
  • Utilizes data to complete program’s key statistical reports and generates reports as needed for the S/VPO, VPCE, program director and client hospital leadership when requested. Responds to requests for general information about the Program’s treatment program and provides information about appropriate topic in response to community requests. Informs Program Director, S/VPO and VPCE of any potential adverse publicity.
  • Performs other duties as assigned/required by this position.
