Responsibilities

  • Manage the admission roster of applicants who have been screened and determined to be eligible for services.
  • Collaborate with intake staff on applicants who are appropriate for admission into inpatient services.
  • Work closely with community stakeholders and the applicant regarding preparation for admission.
  • Document updates in WIC-2/WITS as needed on each applicant to reflect current disposition.
  • Make appropriate referrals as needed and recommended to other community-based program for needed services.
  • Provide reports as needed on applicant trends and processes to include recommendations at Care Improvement meetings.
  • Maintains client records in accordance with Hina Mauka policies and procedures and in compliance with all state and federal laws, licensing and accreditation standards and contract stipulations.
  • Completes fiscal, statistical reporting and quality improvement tasks as required.
