Qualifications

• Pharmacist must hold a license to practice in the state of Arizona.
• Experience in home infusion therapy and sterile compounding, including USP 797/800 knowledge.
• Strong communication, confidentiality, and patient-facing skills are essential.

Responsibilities

• Document clinical activities, review orders, conduct assessments, and reconcile medications.
• Supervise sterile compounding, dispense medications, and ensure compliance with laws and USP standards.
• Interact with pharmacy staff, clinicians, and patients to coordinate care and scheduling.
• Support clinical learning, PQC activities, on-call rotations, and licensure processes.
