Job summary

Corewell Health Farmington Hills Hospital is seeking an Infectious Diseases (ID)/Antimicrobial Stewardship Clinical Pharmacist Specialist. The role focuses on ASP growth, maintenance, and documentation in acute and ambulatory settings, collaborating with clinical pharmacy leaders, administration, and medical staff.

Qualifications

• Bachelor's Degree Pharm.D. or Bachelor in Pharmacy required.
• Residency training preferred.
• 3 years of relevant experience.
• 3 years of experience as a clinical pharmacist or residency/fellowship preferred.
• In absence of residency training, candidates with demonstrated superior knowledge in area of focus may be considered.
• Preferred Infectious Diseases residency training, equivalent years of experience, or board certification preferred.

Responsibilities

• Provide professional infectious diseases pharmaceutical care for Corewell Health patients.
• Collaborate with pharmacy leaders, hospital administration, and medical staff to advance the Antimicrobial Stewardship Program (ASP).
• Monitor patients' medication therapy, review lab results and clinical parameters related to ID.
• Promote safe, appropriate, and cost-effective antimicrobial therapy and educate prescribers and staff.

Education

Pharmacy degree (Pharm.D. or B.Pharm)

Residency training (preferred)

Infectious Diseases residency training or board certification preferred
