Job summary

Baystate Health seeks a Clinical Pharmacy Specialist to lead comprehensive pharmaceutical care in inpatient care areas and support education programs. The role includes clinical rounds, drug therapy optimization, and collaboration with care teams to improve patient outcomes.

The Specialist will precept APPE students and residents, develop staff training for prescribers and nurses, and contribute to research and education initiatives across Baystate Health.

Qualifications

• Pharmacist must have PGY-1 equivalent to three years in a direct patient care role.
• PGY-2 experience preferred.
• ASHP residency preceptorship requirements.
• Board Certification in Pharmacotherapy is desirable.
• Licensed Pharmacist in good standing.

Responsibilities

• Provide comprehensive pharmaceutical care for inpatient care areas and support operational aspects of care areas.
• Participate in patient care rounds to monitor therapy, review dosing, interactions and formulary use.
• Counsel patients on discharge medications and participate in emergencies.
• Document services in pharmacy databases and medical records.
• Precept APPE students and residents, and develop staff education for prescribers and nurses.
• Lead and participate in medication-use evaluations and quality improvement initiatives.
