Qualifications include Pharm. D. with PGY-1; PGY-2 in ID/ASP or BCIDP-equivalent, Florida license, and BLS with possible ACLS/PALS.

Qualifications

• Education: Pharm. D. and PGY-1 residency, PGY-2 in infectious disease/ASP, or 2+ years with BCIDP certification.
• Experience: Hospital experience preferred.
• License: Florida pharmacist license required.
• Certifications: BLS within 30 days; ACLS/PALS if code attendance required; BCPS/CGP preferred.

Responsibilities

• Review and monitor patient medication therapy and orders for safety and efficacy; advise the care team and document interventions.
• Perform order verification and medication profile reviews; check for duplications and drug interactions.
• Prepare, label and dispense medications per prescriptions; ensure regulatory compliance.
