Job summary

St. Mary's Medical Center in Florida is seeking a Pharmacist for Infectious Disease/ASP to provide pharmaceutical care, oversee therapy, and ensure documentation and compliance with laws. The role emphasizes patient safety, medication optimization, and collaboration with the care team.

Qualifications include Pharm. D. with PGY-1; PGY-2 in ID/ASP or BCIDP-equivalent, Florida license, and BLS with possible ACLS/PALS.

  • Experience: Hospital experience preferred.
  • License: Florida pharmacist license required.
  • Certifications: BLS within 30 days; ACLS/PALS if code attendance required; BCPS/CGP preferred.

  • Review and monitor patient medication therapy and orders for safety and efficacy; advise the care team and document interventions.
  • Perform order verification and medication profile reviews; check for duplications and drug interactions.

The Pharmacist – Infectious Disease/ASP provides pharmaceutical care services to patients, including oversight, monitoring, preparation, dispensing, and proper documentation of patient medication therapy. The pharmacist also provides drug information, patient information, and oversight of information and dispensing systems, ensures compliance with federal, state, and local laws and regulations, and may advise physicians on medication utilization.

  • Review and monitor patient medication therapy and orders for safety and efficacy, considering specific patient conditions and health status; advise the patient care team and document interventions.
  • Perform order verification and medication profile reviews, checking for duplications, drug–drug and drug–food interactions, allergies, drug appropriateness, dosing, and frequency.
  • Prepare, label, and dispense medications for patients in accordance with physicians’ prescriptions; ensure compliance with federal, state, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records.

Qualifications
  • Education: Required – Pharm. D. and PGY‑1 residency, PGY‑2 in infectious disease/ASP, or at least 2 years of equivalent experience with BCIDP certification.
  • Experience: Preferred – Hospital experience.
  • License: Required – Current registered pharmacist license in the State of Florida.
  • Certifications: Required – BLS within 30 days of hire (ACLS and/or PALS if code attendance required). Preferred – Board Certification in Pharmacotherapy (BCPS), CGP, etc.
